Dear Sir or Madame, My name is .
I wish to request NOT to disclose any of my credit report information to any creditors effective immediately. My social security number is . Please let me know if this is possible or if you need any additional information in order to grant my request. Thank you very much for your immediate attention.
Best regards,

As of right now, that's not possible, but beginning January, 2003, victims of fraud and identity theft in the state of California will be able to request a Security Freeze which will cause the credit report to be withheld if anyone tries to get credit using the victim's identity. It's expected that this will eventually be available nationwide by legislation or simply the policies of the credit bureaus, themselves.
Unfortunately (if you are trying to hide something), a security freeze will not keep existing creditors, child support authorities, the courts, or the tax collectors out of your files. In fact, there's even a "prescreen" exemption - junk mailers will still get the benefit of your credit record.
