The SPA-3000 is now working, no I'm not sure what I did differently this time.  I can place calls from the cordless phone, and receive calls from the PSTN.  There are a few minor issues to be worked out.  The distant end reports an echo when using the cordless phone with VoIP, and the cordless phone doesn't ring when a call is directed to it.  It can answer the call, but  it doesn't ring.  I'll have to dig further into it.  I also discovered a feature in the SPA-3000.  If it doesn't have a properly formatted CID it will not answer the phone, so I don't have to build that into my asterisk config since the SPA does it by default.  Another interesting feature was that when the cordless battery died it did not hang up the line, and had to be disconnected.  This is probably in the cordless phone itself, and not the VoIP system, but it is interesting none the less.
