Michael Bridgen posts in a Lambda the Ultimate discussion thread that they've updated their 'Scheme in the Real World: A Case Study' document. This paper outlines how the NMK website was implemented using Cocoon, XSLT, J2EE and Sisc Scheme. They used Sisc to implement a continuation based web framework and they have some good detail in their updated paper.

More great information from Avi Bryant about the internals of Seaside. In that post Avi writes about how continuations are expired and the filtering facility that does this as well as other things like HTTP Authentication.
