Noise Ninja is about to give Grain Surgery a run for their money in the Mac market. It's about time too. Grain Surgery, a Photoshop plug-in that removes unwanted noise, works fantastically on many images, but it does a rather mediocre job on images shot at high sensitivities, say ISO 1000 and above.
Sometimes though, you just need to embrace the noise!
