I switched from notebooks to 3 ring binders to put my client notes in, well in the last few months of 2005. I just discovered another advantage: being able to add random things (like pages from my phone notebook) directly into the client notebook. Just punch the paper, open the 3 rings, and add it!

Disadvantage: rings always seem to be on the side, never the top (where I like them)
