I was sick of hunting around with a mouse to find the little icon to disconnect a dial-up connection.  I found that if you make a shortcut to the program rasdial.exe, you can tell it to disconnect via a keystroke.  Right click on the shortcut icon and select properties.  For Shortcut key I put CTRL-ALT-0.  In the target append /DISCONNECT right after the program name:

C:WINNTsystem32rasdial.exe /DISCONNECT
